There isn't much that makes Naoto actually look confused. This is probably at the top of the list. Yamato-Takeru goes into a defensive stance for a moment before Naoto waves a hand at him.

"Adachi-san." She looks at him for a moment, eventually speaking. "From your manner towards me, I must assume we are from different universes." Her tone is not necessarily cold, just confused and dry.
